Return Staff Policy: Permanent staff must be separated from Providence for a minimum of
3 months to return as a traveler.
Travelers can only work for a maximum of 1 calendar year in their assignment. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility.
Facility only accepts Hep B vaccines and will not take titers or declination
REFERENCES - Minimum of 2 supervisory references covering 1 year (6 months for CNA s) of employment total
within the last three years within a comparable specialty/setting that they are booked. This can include Charge RNs / Leads.
If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
Housing: Please have your traveler look into housing options in the area, as options may be limited

Modules are non-billable, average 16 hours spent. Over 20 hours requires PM approval
